1. Persons inquiring about Footprints in Time will be asked to provide basic information about the prospective participant to determine appropriateness of placement. 2. A prescreen questionnaire will be used to gather information about the potential participant's needs and abilities. This screen can be done on the telephone. 3. Arrangements will then be made for an initial tour and visit to the day program. This visit needs to include the potential participant and the caregiver/responsible party.

4. Plan to spend at least an hour on the initial visit. The potential participant is encouraged to take part in the program and the caregiver will complete the necessary paper work.     5. After the initial visit a staff member from Footprints in Time (FIT) will call to inform the potential participant and responsible party of the results of the assessment process. At that time, a schedule can be set up for participation.
